Your First Visit
1) Request Appointment
If you have not yet scheduled an appointment, you may do so by requesting an appointment online or by calling our Upper St. Clair office at (412) 221-2221 or Castle Shannon office at (412) 343-4100.
2) Download and Print New Patient Form
After you schedule your appointment, we encourage you to download and print our new patient form, so you can fill them out at your convenience. You can email this to us at manalosmiles@aol.com or bring it with you to your first appointment. You also may complete the form at our office. Please arrive at least 10 minutes prior to your schedule appointment time to complete the form in our office.
3) Insurance
Please be prepared to bring a copy of your dental insurance card or information with you. We generally need the following: Subscriber information, social security number or insurance ID number, group name or number, and the name of the dental insurance company along with a claims submission address and phone number.
4) Get Directions
If you are unable to get directions on our location page, you may call our office to get more detailed directions.
